User must have a Confidentiality Statement on file
Supervisory approval is required
New User: Complete and fax to 744-9944
Existing User (Update/Reactivate/Deactivate):
Enter a request through the ITCS SDS Service Request System

To request ftp access to the IDX system

Supervisory approval is required
Complete and fax to 744-9944

All other requests (Service Request, New Report Request, Dictionary Update, System Investigation)
should be made using the ITCS SDS Service Request System
