IMPORTANT PERSEUS INFORMATION

1.  Anonymity and Confidentiality – Perseus “External Email List” does not give you anonymity. If you are planning to use an “External Email List” (the process whereby you import email addresses for respondents into Perseus) so that you enable Perseus to send invitations, respondents’ data will include their userid when downloaded in CSV, XLS (Excel) and TVS format.  It will not be included when downloading in SPSS.SAV format. Your opening instructions in your survey should include a statement about anonymity and confidentiality.

2.  If you are a graduate student doing thesis or dissertation research, working as research assistant or teaching assistant you must be sponsored by a faculty member.Your faculty member must fill out this form and list you as a consulting member of a research project.

All studies must conform to standards set by FERPA and HIPAA. Data about a person’s identity must be kept strictly confidential and must not be used. This includes, but is not limited to the following list.

My study does not include:

• Social Security Number (SSN)
• credit card number
• driver's license number
• personally identifiable patient information
• personally identifiable student information
• personnel information
• proprietary research data
• confidential legal data
• proprietary data that should not be shared with the public
